Botulinum toxin can change brow height and make upper-eyelid skin look better or worse, so it can change a blepharoplasty plan. The effect depends on which muscles were treated, the dose, diffusion, baseline asymmetry and how much the patient used the frontalis to hold the brow up before injection.
Why the eyelids may feel heavier after Botox
The frontalis elevates the brow. A patient with brow descent, excess upper-lid skin or true eyelid ptosis may recruit this muscle throughout the day. If toxin weakens that compensation, the brow can settle and previously hidden hooding becomes visible. That does not mean Botox created new skin; it revealed the position the brow assumes without constant muscular elevation.
Injection of brow depressors can have the opposite effect and alter brow shape. A randomized clinical study found measurable changes in brow height and morphology after different depressor injection patterns. This is why photographs taken while toxin is active may not represent stable anatomy.
Four diagnoses can produce similar hooding
| Finding | Main question | Why it matters |
|---|---|---|
| True skin excess | How much skin can be removed without impairing closure? | Over-resection can worsen dry eye and lagophthalmos |
| Brow descent | Is the low brow creating the apparent excess? | Blepharoplasty alone may leave persistent heaviness |
| True eyelid ptosis | Is the eyelid margin itself low? | Selected ptosis correction addresses a different structure |
| Upper-orbital hollowing | Has fat, bone support or prior surgery reduced volume? | A subtractive operation may deepen the hollow |
The examination should include brow position at rest, forehead recruitment, eyelid-margin height, levator function, closure, ocular surface symptoms and old photographs. The decision is based on the structures involved, not on the amount of skin visible in a single image.
What to document before consultation
- Product name, date and approximate dose.
- Injection map or treated areas, including the forehead and brow depressors.
- Photographs before and after treatment.
- Previous heaviness, asymmetry, ptosis, dry eye or contact-lens intolerance.
- Fillers, threads, biostimulators, energy devices and prior eyelid or brow surgery.
There is no single waiting interval for every patient
Surgery does not have to be delayed by a fixed number of months solely because toxin was used. The relevant question is whether the brow and eyelid examination represents a predictable baseline. If brow movement is still changing, if the pre-injection position is undocumented or if toxin-related ptosis is present, waiting for the effect to recede can improve diagnostic accuracy.
Attempting repeated early toxin “corrections” can create a second imbalance. New visual symptoms, a low eyelid margin or a marked asymmetry should be examined rather than managed from photographs alone.
How this affects Hybrid Blepharoplasty
Dr. June Favarin’s Hybrid Blepharoplasty is a diagnostic framework rather than a fixed package. Upper-lid skin is treated conservatively, with attention to orbicularis preservation, brow position and closure. Lower-lid bags, hollowing, support and skin quality are assessed separately. Microfat, nanofat, canthopexy, selected ptosis repair or video-assisted brow surgery are added only when their target problem is present.
Video-assisted brow surgery does not replace blepharoplasty automatically. It addresses brow and upper-face position through scalp incisions and carries separate risks, including numbness, asymmetry, fixation problems, hairline or scar concerns and temporary or persistent motor weakness.
Safety and recovery
Blepharoplasty risks include bleeding, infection, dry eye, chemosis, asymmetry, difficulty closing the eye, lid retraction, ptosis and rare visual complications. Recovery is usually measured in weeks, while scars, swelling and small asymmetries continue to mature for months. The brow can also change as edema resolves and habitual forehead recruitment returns.
International patients should arrive with their injection history and plan enough time in Brazil for the preoperative examination and postoperative checks. Belvivere coordinates hospital care, anesthesia, local transport and follow-up based on the final surgical scope.
