Deep Neck surgery treats selected structures beneath the platysma; it is not a stronger version of liposuction and it is not required in every neck lift. The indication depends on skin, superficial and deep fat, platysma behavior, digastric contour, submandibular glands, mandibular support, chin projection and hyoid position.

Three operations reach different levels

ApproachMain targetPrincipal limit
Submental liposuctionAccessible superficial fat above the platysmaDoes not repair muscle or reach deep structures
Neck LiftPlatysma support, selected superficial fat and skin redrapingCannot change skeletal anatomy or hyoid position
Deep NeckSelected subplatysmal fat, digastric contour and other diagnosed deep contributorsAdds dissection and structure-specific risk

A sharply defined cervicomental angle cannot be produced by a single maneuver in every patient. A retrusive chin, short mandibular body or low hyoid can limit the result even after technically complete soft-tissue surgery. These distinctions are explained further in the comparison of Neck Lift and Deep Neck.

How Dr. June structures cervical support

When indicated, Dr. June Favarin combines central platysma plication with lateral support rather than relying on central skin or muscle excision. Her central repair uses an overlapping vest-over-pants configuration. Lateral suspension creates a hammock that supports the cervical contour and connects the central correction with the mandibular line.

Mandibular retaining ligaments may be released subcutaneously when they contribute to jowl tethering and loss of jawline continuity. These maneuvers address position and support. They do not require automatic removal of every structure exposed through the submental incision.

Which deep components may change the plan?

Subplatysmal fat

Deep fat can contribute to central fullness after superficial fat has been reduced. Conservative removal may improve contour, but aggressive reduction risks irregularity, adherence and an over-skeletonized neck.

Anterior digastric muscles

Prominent or asymmetric anterior digastric bellies can affect the submental shape. Selected shaving or contouring may be considered. A cohort of 452 neck-lift patients found no increase in prolonged dysphagia, dysarthria or throat pain with anterior digastric myectomy, but the study was retrospective and many patients had associated gland treatment. It does not make digastric reduction routine.

Hyoid-related support

The hyoid’s position influences the depth and angle of the neck. Selected fascial release, digastric manipulation or a support sling may change the soft-tissue relationship around it, but surgery does not relocate the bony anatomy without limits. Promises of a fixed angle are inappropriate.

Submandibular glands

Partial gland reduction is optional. It may help selected lateral submandibular fullness, particularly when the gland remains prominent after other causes are addressed. A 2025 review reported hematoma, sialocele, salivary fistula, nerve injury and neck induration across published series. The evidence was heterogeneous and largely observational.

Gland reduction can also cause salivary or dry-mouth symptoms. A patient may reasonably decline it, accepting some residual prominence. Deep Neck should not be defined by whether a gland is treated.

Previous procedures can alter the plane

Submental injections, fillers, threads, energy devices and prior liposuction can produce inflammation or fibrosis. Patients should provide product names, dates and treatment locations. High-frequency dermatologic ultrasound may help map uncertain material. PMMA or liquid silicone can substantially change risk and may contraindicate elective surgery.

Risks and recovery

Risks include hematoma, airway compression, infection, seroma, skin compromise, contour irregularity, scar, altered sensation, salivary problems and temporary or persistent nerve weakness. Lower-lip or tongue-related dysfunction, swallowing symptoms and voice change require examination. Sudden swelling, pressure or breathing difficulty is an emergency.

Swelling, firmness, bruising, tightness and numbness evolve over weeks to months. Early contour does not represent the final result. Major Neck Lift and Deep Neck surgery are performed in hospital under general anesthesia with postoperative monitoring.

Planning for patients traveling to Brazil

Remote photographs can identify broad concerns, but final selection requires palpation and dynamic examination in Brazil. Patients generally plan at least 15 days locally after major facial and neck surgery, adjusted to recovery. Belvivere coordinates hospital logistics, accommodation support, transport and scheduled review; flight clearance remains a medical decision.
