Loss of jawline definition can result from jowls, skin laxity, facial volume changes, platysma descent, neck fullness or limited chin and mandibular projection. A “mandibular Deep Plane facelift” is not a standardized solution for all of these findings. Treatment begins by identifying which structures create the contour.

What Deep Plane may contribute

Deep Plane facelift releases selected retaining ligaments beneath the SMAS so deeper lower-face tissues can be repositioned, often with a more vertical vector. It may improve jowls and the soft-tissue border of the mandible. It does not sharpen every jawline, and a deeper dissection is not automatically better than an appropriate SMAS technique.

The jawline and neck must be assessed together

Lower-face lifting alone may leave platysma bands, skin laxity or deep neck fullness untreated. Conversely, aggressive neck treatment cannot compensate for significant lower-face descent or limited skeletal projection. Deep Neck maneuvers and partial submandibular-gland reduction are reserved for selected anatomy and add specific risks.

Risks and limitations

Risks include hematoma, infection, skin-healing problems, visible scars, asymmetry, prolonged swelling, sensory changes and temporary or permanent motor nerve dysfunction. Marginal mandibular and cervical nerve branches are particularly relevant around the lower face and neck. Perfect symmetry or a specific jaw angle cannot be guaranteed.

International-patient planning

Previous fillers, threads, devices and surgery may alter the plan; dermatologic ultrasound can be helpful. Belvivere coordinates procedures at Hospital São José and provides international concierge support. Major facial surgery generally requires at least 15 days in Brazil, adjusted to recovery.
