Direct answer
Most patients should not stop a GLP-1 medication automatically before
facial surgery, but every patient must disclose the drug, dose,
schedule, last injection and digestive symptoms to the surgical and
anaesthesia teams. Patients in dose escalation or with nausea, vomiting,
bloating, constipation or suspected delayed gastric emptying may need a
24-hour liquid diet, medication adjustment, ultrasound assessment or
postponement.

Why do GLP-1
medicines matter to anaesthesia?
Semaglutide, tirzepatide and related medicines can slow gastric
emptying. Food remaining in the stomach during general anaesthesia or
deep sedation may increase the risk of regurgitation and aspiration into
the lungs.
The risk is not identical for everyone. It is influenced by the
medicine, dose, recent dose increases, gastrointestinal symptoms, other
medical conditions and the planned procedure. That is why an old
universal “stop for one week” rule has been replaced by a more
individual, multidisciplinary approach.
Which details
should I send before travelling?
Provide the brand and generic name, dose, weekly or daily schedule,
date of the last dose and when the dose was last increased. Report
nausea, vomiting, early fullness, reflux, abdominal pain, bloating and
constipation — even if they seem mild.
Also list diabetes, previous gastric surgery, gastroparesis, reflux
disease and all other medicines. Do not wait until admission in Brazil;
the information can change preparation and travel timing.
| Information | Why it matters |
|---|---|
| Current dose and schedule | Helps identify pharmacologic exposure |
| Dose-escalation phase | Symptoms and delayed emptying may be more likely |
| Digestive symptoms | May signal higher residual gastric content risk |
| Diabetes treatment | Interruption can affect glucose control |
| Previous stomach surgery | May change emptying and aspiration risk |
| Rapid weight loss | May affect nutrition, muscle and facial volume |
Will I be asked to stop
the medication?
Possibly, but not automatically. Multi-society guidance indicates
that many low-risk patients can continue. Higher-risk patients may
require a liquid-only diet for 24 hours, a modified anaesthetic plan,
point-of-care gastric ultrasound when available, temporary interruption
or rescheduling.
Stopping a diabetes medicine without a plan can raise blood glucose.
Continuing despite significant symptoms can also be unsafe. The decision
should be made by the prescribing clinician, surgeon and
anaesthesiologist — not by social media or the patient alone.
What counts as a
clear or liquid-only diet?
Use the exact written instructions from the team. A liquid-only day
and the final fasting period are not the same thing. Milk, smoothies,
protein shakes, soups with solids and drinks containing pulp may not
qualify for the final clear-liquid window.
If you misunderstand or break the instructions, tell the team. Honest
disclosure may lead to a delay; concealment can create preventable
risk.
Why does
rapid weight loss matter to facial planning?
Significant ongoing weight loss can change facial fat compartments,
skin support and neck contour after the consultation. It can also reduce
protein and micronutrient intake. Surgery planned on a rapidly changing
face may no longer match the anatomy months later.
Patients do not need an arbitrary “perfect weight,” but stability and
adequate nutrition help make planning more meaningful. A responsible
surgeon may recommend waiting if weight is still changing quickly or
intake is inadequate.
What
should international patients arrange in Brazil?
Plan enough time for in-person assessment, anaesthesia review and any
requested tests before surgery. Do not schedule arrival the night
before. Bring medication in original packaging and carry a concise
medical summary.
Your local stay must also allow postoperative reviews. A procedure
should never be fitted between flights as if it were a tourism
appointment. International care requires a plan for recovery,
communication and follow-up after you return home.
When might surgery be
postponed?
Active vomiting, severe nausea, marked bloating, inability to
tolerate intake, uncontrolled glucose, concerning gastric findings or
failure to follow preparation may lead to postponement. This is a safety
decision, not a punishment.
FAQs
Should I
skip my weekly injection before flying to Brazil?
Do not change it without instructions from your prescriber and
Brazilian surgical team.
Does a
24-hour liquid diet guarantee an empty stomach?
No. It is one risk-reduction measure; symptoms and individual factors
still matter.
Can gastric ultrasound be
used?
In selected cases and where expertise is available, point-of-care
ultrasound may help assess stomach contents. It is not required for
every patient.
Can I have
surgery while actively losing weight?
Sometimes, but rapid ongoing change may affect nutrition and facial
planning. Candidacy is individual.
What
if I use GLP-1 for diabetes rather than weight loss?
Glucose management becomes especially important. Never interrupt
diabetes treatment without a coordinated plan.
