How Long Does Facial Fat Grafting Last?

Facial fat grafting can provide long-lasting volume because the cells that establish a blood supply become living tissue in the recipient area. However, not all transferred fat survives, and no surgeon can promise an exact retention percentage or a fixed duration.

What happens after fat transfer?

Fat is harvested from the patient, prepared and placed in small amounts through fine cannulas. During the early weeks, the graft depends on diffusion and then develops new vascular connections. Part of the transferred volume is reabsorbed during this period.

Initial swelling can make the face look fuller than the eventual result. The contour becomes more reliable over the following months, which is why early photographs should not be treated as the final outcome.

Does surviving fat remain permanently?

Fat that integrates can remain for many years, but it continues to behave like fat elsewhere in the body. It may enlarge with weight gain, decrease with weight loss and change as the face continues to age. “Permanent” should therefore not be interpreted as unchanged for life.

What influences retention?

  • Recipient-site blood supply and scar tissue
  • Previous surgery, fillers, threads or energy treatments
  • Harvesting, processing and placement technique
  • The volume placed and the capacity of each tissue plane
  • Smoking, health conditions and healing quality
  • Weight stability after surgery

Overfilling is not a safe way to compensate for uncertainty. Larger deposits have less contact with vascularized tissue and may increase the risk of irregularity, oil cysts or fat necrosis.

Microfat and nanofat are not interchangeable

Microfat contains small fat parcels used for structural volume in areas such as the temples, orbital contour and lid–cheek junction. Nanofat is more finely processed and used superficially for selected skin-quality indications; it does not provide reliable structural volume or lift.

Previous injectables and imaging

Prior fillers and biostimulators can affect contour and tissue behavior. Dermatologic ultrasound may be indicated to identify material before facial surgery. Hyaluronidase can modify hyaluronic-acid filler, but its effect varies and complete removal cannot be guaranteed.

Risks and possible refinements

Risks include bruising, swelling, infection, asymmetry, undercorrection, overcorrection, contour irregularity, palpable nodules, oil cysts, fat necrosis and rare vascular complications. Some patients may benefit from a later refinement after the tissues have stabilized; this possibility does not mean the first procedure failed.

Recovery and international travel

Swelling and bruising are expected and can be asymmetric. Social recovery often takes several weeks, while smaller changes continue for months. International patients should not plan important events immediately after surgery.

Belvivere generally recommends arriving in Criciúma one or two days before surgery and remaining in Brazil for at least 15 days. Surgery is performed in a hospital setting, with logistical support from the concierge team.
